Middle School Health Screenings

In accordance with the new Nebraska State Statute, #173 NAC 7, Millard Public Schools is required to provide health screenings to 7th grade students enrolled in our district. Health screenings are not intended to be diagnostic, but to identify students with potential problems. 

The required screenings include, vision (distance), hearing, height, weight, and dental screenings. 7th grade students who have not had these screenings completed during their 7th grade physical or during school orientation will be screened at their school. Screenings will be conducted during the months of October and November.

The state statute requiring health screenings in Nebraska does not allow a waiver option. Parents can request that their child not be screened at school but, parents/guardians must have their medical professional complete the Parent/Guardian Exception Form and an Affidavit of Parent/Guardian Refusal of School Health Screenings Form.  The student’s screening by a medical provider must be provided to the school health office by December 1, 2015. If a completed Parent/Guardian Exception Form and an Affidavit of Parent/Guardian Refusal of School; Health Screenings Form from a medical provider are not provided by December 1, 2015, the child will be screened at school. The cost of screening by an alternate provider is the responsibility of the parent/guardian.
